👏 Congratulations to Impact Health for the tremendous impact they’re making in North Carolina! (No pun intended). North Carolina’s Healthy Opportunities Pilot (HOP) is the nation’s first comprehensive pilot program to evaluate the impact of addressing health-related social needs (HRSNs) for high-needs Medicaid enrollees. 🫂 Harnessing Unite Us technology, the NC Department of Health and Human Services is streamlining referrals, authorizations, enrollment, and reimbursement for services through the statewide network, NCCARE360. 🌐 What does that look like in terms of improving community health? 👀 —> 30K+ enrollees —> 560K+ services delivered —>95% invoice acceptance rate (*as of August 31, 2024) 📊 Check out the numbers for yourself —> https://lnkd.in/etDWP4KP 🤝 Learn more about the origin of our partnership —> https://lnkd.in/eG5Vbzar

When discussing #SDoH, the words we choose can make a big difference in patient care. In this podcast clip Dr. Halima Ahmadi-Montecalvo, PhD, MPH, Associate VP of Research and Evaluation at Unite Us, shares insights on the evolving language in healthcare and why it matters. Using the right language helps us deliver care that’s truly centered on the patient, making a positive impact on their health outcomes. Here's why it matters: 🚀 Shift from Social "Determinants" to Social "Drivers" 🚀 Moving away from "determinants" highlights that these factors aren't set in stone—they can be influenced and changed. 💪 Empowering Patients 💪 The right language gives patients the power to manage their health and well-being, rather than feeling trapped by circumstances. 🎯 Focusing on Individual Needs 🎯 Addressing specific social risks, like housing or food insecurity, is crucial, but so is recognizing what patients say they need most. Although the term "determinants" is still widely referenced and used in the healthcare space—these conversations are pivotal for driving the industry forward and delivering more equitable care. Gain more insights! 🎧 You don't want to miss out on this conversation led by American Hospital Association(AHA) VP of Business Management and Operations, Kathleen Wessel. 👉 Listen to the podcast here: https://bit.ly/3LCNMut
